Historical and modern place references in the corpus

Note: coordinates come from a curated historical gazetteer, not a geocoding service. A place marked “Named” was known to the gazetteer by that spelling; “Rule-matched” means a prefix or substring rule chose the place on our behalf and may have chosen wrong. “Unplaced” means nothing matched, so it has no coordinates and never appears on the map.
